Intake & Enrollment

What Is the Intake Process?

Intake is the first step to receiving home care through Good Care Agency. Our specialists talk directly with patients and their families to understand the situation, explain the options available, and begin the enrollment process — handling all paperwork on your behalf.

What Happens During Intake

  • Initial consultation by phone or in person
  • Assessment of medical history, physical and mental state
  • Review of special requirements and preferences
  • Explanation of services available and coverage options
  • Medicaid enrollment assistance if needed
  • Caregiver matching based on language, skills, and location
  • Care schedule coordination

Most families are surprised by how fast we move. Once intake is complete, we can often have a caregiver in place within the same week — sometimes sooner.

Languages We Speak

Our intake team speaks English, Russian, Spanish, Ukrainian, Uzbek, and several other languages. You should never have to navigate this process in a language that isn't yours.

1. How do I apply for home care services?

We can help you get started. We'll ask a few questions about the patient's health, daily needs, and insurance to determine eligibility and the right type of services.

2. Do I need a doctor's referral to start care?

No. You do not need a doctor's referral to get started. If any notes or documents are needed, we'll let you know and help you through those steps.

3. How quickly can services begin after enrollment?

In many cases, services can begin within a few days. The timeline depends on completing intake, receiving required documentation, and scheduling the assessment.

4. What is the sign-up process?

We collect basic information about the patient's insurance and needs, verify insurance, and help schedule the necessary in-home or virtual assessments.

5. Do you accept Medicare and Medicaid?

Yes, we work with both. We'll check your insurance and walk you through what services you may qualify for.

6. Can I receive home care only under Medicare?

Yes, but Medicare coverage is limited. We can check what Medicare covers and also explore other options that may be available.

7. Do you work with the Holocaust Survivors Program?

Yes. We help check eligibility, guide through the application process, and support you every step of the way.

8. Can you help patients with dementia, Alzheimer's, or those who are bedbound?

Absolutely. We'll guide you through the steps and explore Medicaid programs like NHTD and TBI that help patients stay home safely instead of going to a nursing facility.

9. What is the NHTD program?

A Medicaid program that helps patients stay home safely with personal care and community support services. Patients can receive up to 24/7 hours of care under this program.

10. Can you help with transitioning into the NHTD program?

Yes, we assist with the entire process — eligibility checks, application, and coordinating all services needed for a smooth transition.
