Pay & Benefits FAQ
Everything about your paycheck, PTO, benefit cards, and retirement.
1. What is the pay rate?
$19.65 per hour, plus $1.92 on the Benefit Card (Clarity or FBA).
2. What is the overtime rate?
$29.48 per hour for any hours worked over 40 in a week.
3. What is the payroll schedule?
The work week runs Sunday through Saturday. Payout day is Friday of the following week.
4. How do I receive my payment?
By paycheck sent by mail, or by direct deposit to your bank account after you provide your bank details.
5. How can I see my paystub?
Through the Viventium app. We send you a registration link — follow the steps to create your account and access your paystubs.
6. What is the difference between gross pay and net pay?
Gross Pay is the total amount you earn before any taxes or deductions. Net Pay is the amount you actually receive after all deductions are subtracted.
7. How does PTO work?
You earn 1 hour of PTO for every 30 hours worked, up to 80 hours per year. Submit the PTO request form to use it.
8. Where can I use the Clarity benefit card?
The Clarity card can be used for cellular communication, transportation, and certain pharmacy purchases. The FBA card is for groceries. Payments are issued once a month covering two months prior.
9. What is the 401(k) deduction?
After 3 months of employment, 3% is automatically deducted for your retirement savings. To opt out, complete the opt-out form in the office or call ADP Retirement Services at 800-695-7526.
10. How do I make changes to my W-4 form?
Come to the office and complete a new W-4 form. We will assist you.
Employment Requirements

1. What are your office hours?
Monday to Friday, 9:00 AM to 5:00 PM.
2. What documents do I need to apply?
One main form of ID (US Passport, Certificate of Naturalization, Permanent Resident Card, or Employment Authorization Card — original); one photo ID (Driver's License or ID Card — original); Social Security Card (original); HHA/PCA Certificate if you have one; two reference letters; and medical documents.
3. What medical documents are required?
Physical exam form signed and stamped by a doctor (not older than 6 months); drug test (not older than 6 months); Rubella titer or vaccine; Measles titer or vaccine if born on or after 1/1/1957; and one of: Quantiferon lab report (negative), chest X-ray, or 2-step PPD.
4. Do I need to complete In-Service training?
Yes. HHA certificate holders must complete two parts (12 hours combined) — Part 1 due by June, Part 2 by December. PCA certificate holders complete one part (6 hours) due by June.
